Contaminated Olives

FDA Urgently Warns Consumers about Health Risks of Potentially Contaminated Olives

The U.S. Food and Drug Administration (FDA) is alerting consumers to possible serious health risks from eating olives that may be contaminated with a deadly bacterium, Clostridium botulinum. C. botulinum can cause botulism, a potentially fatal illness. The olives are made by Charlie Brown di Rutigliano & Figli S.r.l, of Bari, Italy and are being recalled by the manufacturer. No illnesses have been reported to date in connection with this recall.

The olives should not be eaten alone or in other foods, even if they do not appear to be spoiled. Consumers should discard these products or return them to the point of purchase. If in doubt, consumers should contact the retailer and inquire whether its olives are part of the recall.

The olives are sold under the following brands: Bonta di Puglia, Cento, Corrado's, Dal Raccolto, Flora, Roland and Vantia, and have codes that start with the letter "G" and are followed by 3 or 4 digits. All sizes of cans, glass jars and pouches of Cerignola, Nocerella and Castelvetrano type olives are affected.

Symptoms of botulism include general weakness, dizziness, double vision, trouble with speaking or swallowing, difficulty in breathing, weakness of other muscles, abdominal distension and constipation. People experiencing these symptoms should seek immediate medical attention. Consumers may also report illnesses associated with consumption of these olives to the nearest FDA district offices.

Charlie Brown di Rutigliano & Figli S.r.l, initiated a recall of these olives on March 27, 2007. The recalled olives had been distributed to wholesalers, who have marketed them nationally to restaurants and retail stores. FDA concluded that additional warnings are needed because, to date, the company has not contacted importers with specific instructions on the recall.

In addition to re-emphasizing its warning to consumers, FDA is making the following requests:

  • Importers of these olives should discontinue distribution, isolate held stocks and notify customers to take similar actions to prevent the products from reaching consumers. Importers should contact their local FDA office for assistance in implementing the recall.
  • Food manufacturers who have repacked the olives for sale under different names or who have used them in the production of other food should contact their local FDA office.
  • Restaurants, delicatessens, and other food service providers should discontinue using the olives, dispose of their opened containers and contact their suppliers for instructions on what to do with unopened containers.

Raw Milk Butler County Dairy

Agriculture Department Warns Consumers of Tainted Raw Milk Sold by Butler County Dairy

FOR IMMEDIATE RELEASE -- Harrisburg, PA -- April 13, 2007 -- Consumers who have purchased raw milk from Fisher's Dairy, Butler County, any time after April 9, 2007, should discard it immediately due to the risk of Listeria monocytogenes contamination, Agriculture Secretary Dennis Wolff said today.

"During routine inspection, a preliminary test showed the presence of Listeria bacteria in some of the raw milk samples taken from Fisher's Dairy," said Wolff. "If consumers have raw milk from this farm, they should discard it immediately."

There have been no illnesses reported because of the potential contamination, but if individuals who consumed the raw milk become ill, they are advised to consult their physician.

Raw milk is milk that has not been pasteurized or homogenized.

The Department of Agriculture has suspended sales of raw milk at the dairy and is ensuring that corrective action is taken. Samples taken from the farm during routine testing on April 9 tested positive for Listeria on April 13.

Multiple laboratory samples must come back negative before sales can resume.

Symptoms of Listeriosis are fever, muscle aches, and sometimes gastrointestinal symptoms such as nausea or diarrhea. If infection spreads to the nervous system, symptoms such as headache, stiff neck, confusion, loss of balance or convulsions can occur.

Infected pregnant women may experience only a mild, flu-like illness, but infections during pregnancy can lead to miscarriage or stillbirth.

Symptoms of Listeriosis can appear in four days to three weeks.

FOR IMMEDIATE RELEASE -- RARITAN, NJ, April 10, 2007 – Glass bottles of griseofulvin oral suspension (liquid) [GRIFULVIN V®], microsize 125 mg/5mL, a prescription medicine used to treat ringworm and other fungal infections, are being voluntarily recalled as a precaution from wholesalers and retail pharmacies nationwide. The voluntary recall is a precaution based on two reports of glass fragments found in bottles of the liquid formulation. Consumers who believe they are in possession of recalled product should contact the pharmacy where the medicine was purchased.

The voluntary recall is being conducted by Ortho Dermatological, Division of Ortho-McNeil Pharmaceutical, Inc., manufacturer of GRIFULVIN V® and griseofulvin oral suspension, which has a Patriot Pharmaceuticals, L.L.C., label. This voluntary recall is limited to the liquid formulation of the medication and does not include any other dosage form.

The two reports of glass fragments are believed to be the result of bottle breakage during shipping and handling. A plastic over-wrap placed on bottles of this medicine for protection might have made it difficult to detect breakage that occurred during shipping and handling, and action is being taken to change the over-wrap to prevent this possible occurrence in the future. In the unlikely event that a damaged bottle has been dispensed, a potential exists for injury due to accidental ingestion of glass fragments. There have been no reports of adverse events from the reported glass fragments in broken bottles.

The lots were shipped to distributors in the United States only between August 23, 2005 and March 14, 2007. Lot numbers are listed at the end of this press advisory and posted on www.aboutgrifulvin.com. Lot numbers can be found on the back of the product label only on four-ounce (120 mL) glass bottles filled by the manufacturer. Consumers with pharmacy-dispensed bottles, which were filled at the pharmacy and do not contain lot numbers, should contact the pharmacy where they purchased the medicine to determine if they are in possession of product that has been recalled.

In addition to contacting the pharmacy where the medicine was purchased, consumers who believe they are in possession of recalled bottles of GRIFULVIN V® griseofulvin oral suspension (liquid) may also call 1-800-426-7762. Consumers who believe they are in possession of affected product from Patriot Pharmaceuticals, LLC, griseofulvin oral suspension (liquid), may call 1-800-510-0383. Consumers should direct medical questions to their health care providers. Adverse reactions experienced with the use of this product should be reported to the company using the telephone numbers above.

The company is voluntarily conducting this recall in cooperation with the U.S. Food and Drug Administration and sending urgent recall letters to wholesalers and pharmacies nationwide. In addition, the company is taking action to supply new inventory to its customers.

Over the past several days, Menu Foods continued a detailed analysis of production records at its Emporia, Kansas, plant as part of the US Food and Drug Administration’s ongoing investigation of this adulterated wheat gluten. As a result of this work, Menu Foods advises the public:

  • Two additional production dates of eight varieties of pet food have been added to the recall list. These eight varieties of pet food had previously been withdrawn from the market and should already be off the retailer shelves.

Pet Food Recalls

Recall of Pet Foods Manufactured by Menu Foods, Inc.

The Food and Drug Administration (FDA) has been informed that Menu Foods, Inc., a private-label pet food manufacturer based in Streetsville, Ontario, Canada, is recalling all its "cuts and gravy" style dog and cat food produced at its facility in Emporia, Kansas between December 3, 2006 and March 6, 2007. The products are sold in the United States, Canada and Mexico.

The recall was prompted by consumer complaints received by the manufacturer and by tasting trials conducted by the manufacturer. There has been a small number of reported instances of cats and dogs in the United States that developed kidney failure after eating the affected product. To date, Menu Foods, Inc. has reported 14 animal deaths to the FDA. Nine cats died during routine taste trials conducted by the company. Consumers reported deaths of four cats and one dog. The firm has undertaken extensive testing of the pet food products in question, but to date has been unable to find the source of the problem.

The products are packaged in cans and pouches under numerous brand names and are marketed nationwide by many pet food retailers including Ahold USA Inc., Kroger Company, Safeway, Wal-Mart Stores, Inc., PetSmart, Inc., and Pet Valu, Inc.

Menu Foods, Inc. has identified the potentially contaminated products on the Internet at www.menufoods.com/recall. The product listing was expanded and updated as of March 23, and consumers who have any of these products should immediately stop feeding them to their pets. Dogs or cats who have consumed the suspect feed and show signs of kidney failure (such as loss of appetite, lethargy and vomiting) should consult with their veterinarian. Menu Foods, Inc. is notifying retailers by telephone and mail and is arranging for the return of all recalled products.

FDA is conducting an investigation and working with Menu Foods, Inc. to ensure the effectiveness of the recall. Consumers with questions may contact the company at 1-866-895-2708. FDA Warns Consumers Not to Use Certain Jars of Earth's Best: "Organic 2 Apple Peach Barley Wholesome Breakfast Baby Food"

The Food and Drug Administration (FDA) is warning consumers not to use certain jars of Earth’s Best Organic 2 Apple Peach Barley Wholesome Breakfast baby food because of the risk of contamination with Clostridium botulinum, a bacterium which can cause botulism, a life-threatening illness or death. Consumers are warned not to use the product even if it does not look or smell spoiled.

The affected product was sold in single individual jars and in variety packs (which contains 4 jars of the apple peach barley in the pack along with other varieties). The food is part of the firm’s “2nd Vegetables, Fruits and Blends” line intended for babies 6 months and older. The food was distributed through retail stores and sold through the Earth’s Best website. If consumers have any of the specified jars in their home, they should not use it and destroy it immediately. No illnesses have been reported to FDA or the manufacturer to date in connection with this problem.

Botulism, a potentially fatal form of food poisoning, usually causes illness within 18-36 hours of exposure. Initial indication of illness in infants is decreased frequency or absence of stools. Other signs and symptoms noted are poor feeding, weak suck, lethargy, listlessness, weak cry, decreased body tone, and diminished overall movement. Difficulty with swallowing may be evident as secretions drooling from the mouth. This may be followed by decreased respiratory effort which may lead to respiratory arrest from airway occlusion from unswallowed secretions. Older populations who may be consuming the product as part of a pureed diet might experience symptoms such as blurred vision, dizziness, dry mouth, and progressive weakness from head to legs. Constipation and urinary retention are also common. Caregivers or people observing these problems should seek immediate medical attention for those affected.

Hain Celestial Group initiated a recall of the food on February 9th. The recall is ongoing. Production and distribution of the product has been suspended as FDA and the company work to determine the source of the problem. Hain Celestial Group distributed 4,072 cases of the specified individual jars consisting of 24 jars per case and 38,298 variety packs with the specified jars.

Battery Packs for Toy Vehicles by JAKKS Pacific Due to Fire Hazard

Hazard: The lithium-ion polymer batteries used to charge the toy vehicles can ignite while charging, posing a fire hazard.

Incidents/Injuries: JAKKS Pacific has received 33 reports of the batteries melting or catching fire while charging, including three reports of minor burns to fingers and 23 reports of property damage to flooring, seats or walls.

Description: This recall involves battery packs used with FLY WHEELS XPV toy vehicles. The battery packs were sold with a charger (item number 73904) and with the radio-controlled XPV toy (item number 73906). Item numbers are printed on the product’s packaging above the barcode label. The batteries are marked “Fly Wheelz,” “XPV,” and “JAKKS Pacific,” “Lithium-ion Polymer battery (LiPo)” followed by a list of warnings and precautions. The XPV Xtreme Performance Vehicle is not the subject of this exchange program.

Sold at: Toy stores, discount department stores and Internet retailers nationwide from August 2006 through January 2007 for about $30. Battery packs and toy vehicles packaged together sold for between $80 and $90.

Fisher Price Laugh and Learn” Bunny Toys Due to Choking Hazard

Hazard: The pink pompom nose can detach, posing a choking hazard to young children.

Description: This recall involves the Laugh and Learn Learning Bunny that measures about 10-inches tall. The yellow bunny with one green and one orange ear has musical and counting sound effects. The words, “Laugh and Learn” are printed on the bunny’s shirt. Product numbers involved in the recall are: K0468, K2960, K2961, K2962, K2963, K2964, K2965, K3440, K6898, K7884, L0327, and K5862. The product numbers are located on the fabric tag sewn to the body of the bunny. Only bunnies with three dimensional pompom noses are included in this recall. Bunnies with flat or embroidered noses are not subject to this recall.

Sold at: Discount department stores and toy stores nationwide May 2006 through December 2006 for about $15.

Remedy: Consumers should immediately take these recalled toys away from children and contact Fisher-Price to arrange for the return of the bunny to receive a voucher for a replacement toy of the customer’s choice.

Link-N-Lite™ Magnetic Light-Up Puzzles

Name of product: Link-N-Lite™ Magnetic Light-Up Puzzles

Hazard: Small magnets used to connect the puzzle pieces can fall out. Magnets found by young children can be swallowed or aspirated. If more than one magnet is swallowed, the magnets can attract each other and cause intestinal perforation or blockage, which can be fatal.

Incidents/Injuries: The firm has received reports of nine incidents where magnets became loose from these puzzles. No injuries have been reported.

Description: The recalled Link-N-Lite™ Magnetic Light-Up Puzzle, which is a double-sided plastic magnetic puzzle for ages 3 and up, has two designs. The Disney-brand puzzle shows the Disney Princesses on the front and Ariel, from The Little Mermaid, on the back. The Marvel-brand puzzle shows “Spider-Man & Friends” on both sides. Each puzzle measures 8 inches by 10.5 inches, contains nine pieces with four or six magnets per piece, and lights-up when all pieces are in place. A 9-volt battery is placed in the center bottom piece that contains a battery case.

Sold at: Target and Kohl’s stores nationwide and on Amazon.com from June 2006 through January 2007 for about $15.

Remedy: Consumers should immediately stop using the puzzles and contact Jazwares for a free replacement toy that does not contain magnets.

Easy-Bake Ovens Recalled  Due to Entrapment and Burn Hazards

Hazard: Young children can insert their hands into the oven’s opening and get their hands or fingers caught, posing an entrapment and burn hazard.

Incidents/Injuries: Easy-Bake has received 29 reports of children getting their hands or fingers caught in the oven’s opening, including five reports of burns.

Description: The Easy-Bake Oven is a purple and pink plastic oven that resembles a kitchen range with four burners on top and a front-loading oven. “Easy Bake” is printed on the front of the oven. Model number 65805 and “Hasbro” are stamped into the plastic on the back of the oven. The Easy Bake Oven is an electric toy and is not recommended for children under eight years of age. Ovens sold before May 2006 are not included in this recall.

Sold at: Toys R Us, Wal-Mart, Target, KB Toys and other retailers nationwide from May 2006 through February 2007 for about $25.

Remedy: Consumers should contact Easy-Bake between 8:30 a.m. and 4:30 p.m. ET 7 days a week to receive a free retrofit kit with consumer warning. Caregivers should keep the Easy Bake Oven away from children under eight years of age.

Hazard: A small metal zipper tab on the infant booties can detach posing a choking hazard to young children.

Incidents/Injuries: L.L. Bean has received six reports of the metal tab detaching from the booties. No injuries have been reported.

Description: The booties are sold in infant sizes 1-6. The booties were sold in a tan or pink suede. They have rubber soles and a zipper closure at the ankle. The booties are lined with creme-colored faux shearling acrylic/polyester fleece. “L.L. Bean” is printed in white letters on the soles of the booties. The size and stock #BNK1 are printed on a tag inside the booties.

Sold at: L.L. Bean catalogs, Web sites and retail stores in Maine, New Hampshire, Massachusetts, New Jersey, Pennsylvania, Maryland and Virginia from July 2006 through December 2006 for about $16.50.

Remedy: Consumers should take the recalled booties away from children immediately and contact L.L. Bean to receive a full refund.

Hazard: The handle of the curling iron can come apart exposing its line cord, posing a shock or electrocution hazard to consumers.

Description: This recall involves ceramic, gold-matte finish curling irons. The irons have a 1, 1¼, or 1½-inch barrel, metal counter rest, vertically placed ON-OFF buttons, and a heat set dial that ranges from 0 to 30. A four-digit date code can be found on the plug prong. Date codes included in this recall range from November 2005 through July 2006 (ex. 0706). The name “Conair” is printed on the handle of the iron.

Sold at: Discount retailers and drug stores nationwide during January 2006 for about $25.

Remedy: Consumers should immediately stop using the recalled curling iron and contact Conair to receive a free replacement.

Feb 1, 2007            Maytag Recalls Dishwashers Due to Fire Hazard

Name of product: Maytag® and Jenn-Air® brand dishwashers

Hazard: Liquid rinse-aid can leak from its dispenser and come into contact with the dishwasher’s internal wiring which can short-circuit and ignite, posing a fire hazard.

Incidents/Injuries: Maytag has received 135 reports of dishwasher fires, resulting in product and/or property damage. Four injuries have been reported, including three reports of smoke inhalation and one serious hand laceration when operating a fire extinguisher to put out a fire in the dishwasher.

Graco Recalls 100,000 Highchairs for Repair

The high chairs can collapse if not fully opened and locked.

About 100,000 Graco highchairs are being recalled for repair because they can collapse if they are not fully opened and locked into place from the storage position, U.S. regulators said Thursday.

Graco Children’s Products Inc. is recalling the Graco Contempo Highchairs after receiving 18 reports of the chair collapsing. There was one report of an 18-month-old boy suffering a bruise on his foot, the U.S. Consumer Product Safety Commission said.

The highchairs were sold from December 2005 through December 2006 for between $100 and $130 at various retailers nationwide.

 

Geometix  Recalls Magne Blocks

Toys, Ingested Magnets Pose Aspiration and Intestinal Hazards

Distributor: Geometix International LLC, of Brookfield, Conn.

Hazard: Tiny magnets inside the building pieces can fall out. Magnets found by young children can be swallowed or aspirated. If more than one magnet is swallowed, the magnets can attract each other and cause intestinal perforation or blockage, which can be fatal.

Description: All types of MagneBlocks™ construction sets are included in this recall. The sets contain six or more plastic building pieces and six or more 1/2-inch diameter steel balls. Some sets include steel rods. The building pieces are various colors and are shaped in 1 1/2-inch cubes and three-, four-, and five-sided pyramids. The plastic building pieces have “MagneBlocks™” imprinted on them.

Remedy: CPSC recommends children under 6 years of age not play with toys containing magnets. If a magnet comes out of one of the blocks in these sets, immediately remove the block from the set and send it to Geometrix International for a free replacement block

Target Recalls Baby Rattles & Ornaments for Choking Hazard

 

Name of Product: Plush Baby Rattles and Photo Frame Ornaments

Units: About 460,000

Manufacturer: Target, of Minneapolis, Minn.

Hazard: Small parts on the plush rattles and frame ornaments can break or detach, posing a choking hazard to young children. Additionally, the rattles’ plastic ring can break and expose sharp points.

Incidents/Injuries: Target has received 11 reports of the pompoms, eyes, nose and bows detaching from rattles and frame ornaments, including one report of a baby mouthing a detached part. No injuries have been reported.

Description: This recall involves plush rattles and frame ornaments sold in the "See. Spot. Save." department of Target stores. The two styles of rattles include a pink bear and a green moose. The plush animals either have a rattle inside or a plastic ring attached. The photo frame ornaments are a pink bear or green moose holding either a square or heart-shaped frame.

Sold at: Target stores exclusively nationwide from November 2006 through December 2006 for $1.

Remedy: Consumers should stop using the rattles and photo frames immediately and return them to the nearest Target store for a Target GiftCard worth the value of the returned item plus applicable sales tax.

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Children’s Butterfly Necklaces

Units: About 113, 800 (an additional 29, 000 necklaces were recalled in December 2006)

Importer: U.S. Toy Co. Inc., of Grandview, MO.

Hazard: The pendant on the necklace contains high levels of lead. Lead is toxic if ingested by young children and can cause adverse health effects.

Incidents/Injuries: None reported.

Description: The recalled necklaces have pendants shaped as butterflies that are painted in various colors. The pendants hang from a black cord. The necklace’s package is blue cardboard with images of flowers and butterflies and the words "Butterfly Necklace" printed on the front and "Item#JA298" on the back.

Sold at: U.S. Toy Company retail stores and children’s and small discount stores nationwide and online at www.ustoy.com from August 2006 through December 2006 for about $5 per dozen.

Remedy: Consumers should immediately take this jewelry away from children. Consumers should return the recalled jewelry to the store where purchased for a free replacement product, or contact U.S. Toy Company for information on returning recalled necklaces purchased on the Web site

Kids II Inc. Recalls Teethers Due to Choking Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of product: Bright Starts Star Teether Beads and Bright Starts Teether Beads

Units: About 375,000

Distributor: Kids II Inc., of Alpharetta, Ga.

Hazard: The flexible plastic ring that holds the teether beads in place can crack or break, and the beads can detach, posing a choking hazard to infants.

Incidents/Injuries: Kids II has received 24 reports of the plastic ring cracking or breaking and the beads becoming detached from the ring. No injuries have been reported.

Description: The Bright Starts Star Teether Beads (model 8483) has textured soft plastic beads in bright colors and various shapes connected to a flexible plastic ring. The beads on this model are shaped as stars, spirals and ovals. The Bright Starts Teether Beads (model 8549) has plastic beads in bright colors that are shaped as an oval and are connected to a flexible plastic ring.

Sold at: Discount department and juvenile specialty stores nationwide from June 2006 through January 2007 for between $1 and $3.

Remedy: Consumers should take this product away from children immediately and contact Kids II for information on receiving a free replacement.

Children's Hooded sweatshirts with drawstrings Recall

Name of Product: Children’s Hooded Sweatshirts with Drawstrings

Units: About 45,000

Importer: Seena International, of Yaphank, N.Y.

Hazard: The garments have a drawstring through the hood, posing a strangulation hazard to children. In February 1996, CPSC issued guidelines to help prevent children from strangling or getting entangled on the neck and waist by drawstrings in upper garments, such as jackets and sweatshirts.

Incidents/Injuries: No incidents or injuries have been reported.

Description: This recall involves children’s fleece hooded sweatshirts, sizes 8-12. The sweatshirts were sold in black, brown, blue, red, grey, white and off-white. The words “BKLYN Xpress” and “Brooklyn Xpress” and various graphic designs are embroidered on the sweatshirts. A sewn-in tag has one of the following style numbers: BXFL72B, BXFL73B, BXFL74B, BXFL75B, BXFL77B or BXFL80B. This recall only involves sweatshirts in sizes 8 to 12.

Sold at: Various clothing stores, including Ross Stores & Gordman’s, from September 2006 through December 2006 for about $25.

Manufactured in: China

Remedy: Consumers should immediately remove the drawstrings from the sweatshirts to eliminate the hazard or return the garment to the store where purchased for help in removing the drawstring.
